Program Details*:
- • Dentist agrees to purchase a minimum of $3,000 during 12 months of enrollment. When minimum is met, all purchases are eligible to receive an 8% rebate.
- • Minimum first order: $300
- • At the end of the program, dentist receives 8% of the total Alpen purchases in the form of an AMEX Gift Card.
